GP Referral Letter Must Include
1. Patient name, dob, address, contact details, medicare details
2. GP name, provider number, contact details
3. State brief reason for referral under mental health treatment plan
4. Identify the number of sessions as 6 or 4. Medicare will not allow a referral stating 10 sessions.
5. Mental Health Care Plan is in addition to your referral letter
